① Check "Cancel Docomo Mail Portability"
② Tap "Notes" and then check "Agree"
③ Select the destination for the confirmation email
(An address other than @docomo.ne.jp is recommended)
④ Tap "Next".

As the effective date is "Today", it is likely that you will no longer be able to receive emails addressed to @docomo.ne.jp the moment you cancel.
