Actual steps for name change (surname change) procedures at Rakuten Mobile

Here are the actual steps and precautions I took when changing the name (surname) on my Rakuten Mobile account after getting married and my wife's surname changed.

Basically, we will proceed according to this Rakuten Mobile guide.

Required Documents

The required documents are as follows.

1. Rakuten Mobile Name Change (Surname Change) Notification Form
2. Copy of public document confirming the fact of surname/name change

   Any of the following:

   • Driver's license (front and back)
   • My Number Card (Individual Number Card)
   • Certificate of Residence
   • Family Register (full or abstract)
   • Health insurance card
3. Copy of identity verification document
   (Not required if using 'Driver's license' or 'My Number Card' for 2.)

If you use a driver's license or My Number Card, only 1 and 2 are required.

Also, since a public document confirming both the old and new names is required, please complete the procedure after updating the name on your driver's license or My Number Card at the local government office.

Now, let's prepare the necessary documents.

① Prepare the Rakuten Mobile Name Change (Surname Change) Notification Form

The first document, the 'Rakuten Mobile Name Change (Surname Change) Notification Form', can be downloaded and printed from here.

It is recommended to print the name change form on page 1 and the mailing address on page 3.

Print it out and fill in the required information in the red box.

• Phone number
• Date of entry
• Previous name and new name

should be filled in.

② Prepare a copy of the public document

Create a copy of the second public document.

This time, we will use the My Number Card, so we will skip 'Copy of identity verification document' for submission document ③.

As stated in the name change notification form, a public document confirming both the old and new names is required.

Prepare a copy of your My Number Card with both your old and new names, and the preparation of required documents is complete.

Approximately two weeks after mailing the documents, I confirmed that my name on my Rakuten Mobile account had been quietly changed!

Rakuten Mobile offers a family discount, which gives all family members a 110 yen discount. (As of 2026)

It states that common-law marriage is also acceptable, so even couples before marriage can easily apply for the discount from my Rakuten Mobile.
